Procedures of Claim

Step 1: Claimant contact Serendib on claim case via hot line number 088 988 1881 and press number 3.
Step 2: Claimant submits the claim request to Serendib via instructed means of delivery. Serendib will pursue the review and investigation, if necessary.
Step 3: The company shall notify beneficiary in writing about company decision within 15 working days from the date all required documents for claim processing are received in full. Serendib will pay the indemnity to the beneficiary if the claim is approved. Upon settlement of the claim, this contract shall be deemed terminated.

 

Procedures for Providing Information

Before making claim in accordance with this insurance policy, the Company requires the following documents:

  • Claim application form of Serendib Microinsurance Plc., which is filled by the claimant or facilitated by Serendib’s staff or agent and thumb printed by the claimant
  • Insurance Certificate
  • Proof of the insured and claimants’ identification and family book or documents with equal value
  • Report from the police or a local authority stating the date and type of accident
  • Copied of the legal death certificate issued by hospital or local authorities (Commune/Sangkat)(Death case).
  • Certify letter of disability issued by the hospital (Total Permanent Disability case).
